The new system – powered by OCHIN Epic Electronic Health Record (EHR) and funded through generous grants from Kaiser Permanente and Sutter Health – allows CHCN health centers to reliably and confidentially share patients’ electronic health records (EHR) with participating providers.

OCHIN offers a fully hosted, highly customized instance of Epic practice management and electronic health record (EHR) solutions.

In the summer of 2018, Community Health Center Network (CHCN), OCHIN, Kaiser Permanente, and Sutter Health joined forces to form a collaborative that will continue to enhance patient care and coordination to bring the highest quality health care and services to the most vulnerable in Alameda, Contra Costa, and Solano Counties. This new collaboration will enhance continuity of care, supporting whole person care, integrating systems, clinics, hospitals and doctors.

The implementation of OCHIN Epic Electronic Health Record (EHR) will allow CHCN health centers to not only work more collaboratively with specialists and hospitals, but also provide multiple tools for data reporting and population health management. OCHIN Epic offers a patient portal, My Chart, allowing  for better patient engagement and ultimately, improved patient satisfaction.
