Description: A representative from Root & Rebound: Reentry Advocates will join us to present on her organization.  She will go discuss the organization’s mission, its services, and how to connect a patient to Root & Rebound.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
