Description: A representative from Spectrum Community Services will join us to present on his organization and its programs.  He will go discuss what Spectrum programs are offered, including HEAP and how to connect a patient to these services.

Audience: Care Neighborhood community health workers [CHWs], care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
