Description: A representative from American Heart Association will go over community resources for  patients with CHF and discuss ways for CHWs to discuss CHF with patients.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
