Description: A representative from The Bread Project [TBP] will join us to present on TBP job training program.  He will go discuss what program are offered, criteria for patients to qualify for programs and how to connect a patient to these programs.

Audience: Care Neighborhood community health workers [CHWs], care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
