Description: A representative from Beacon Health Options will provide a presentation on her organziation and its programs.  She will go discuss what programs are offered, how to connect a patient to these services, and how CHWs can assist the patient during the referral process.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
