Description: Representative will give overview of Alameda Alliance for Health Dept of Health Education, its services/programs, and how to connect patients to these services/programs.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
