Description: Care Neighborhood staff will go over community resources for senior services available in Alameda County to consider for patients.  Community resources  include senior housing.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
