Description: Care Neighborhood CHW Jing from LMC-DO will lead a training on SMART [specific, measurable, attainable, realistic and timely] goals.  This training is for those who create and work on SMART goals with their patients.

Audience: Care Neighborhood CHWs, care coordinators, outreach workers, and other patient advocates from the eight clinics and CHCN community partners
