Business Analytics for Improved Patient Care

Health center leaders, from CEOs to finance officers, front desk managers, member services and clinical staff, rely on data to provide the best care possible. CHCN’s data is a rich resource, delivering clinical data that health centers may not otherwise receive but are vital to care. We provide business analytics reports for health centers that include information about patient hospitalization, pharmacy and lab use. This data can help providers flag, target and resolve issues for patients with the highest needs. CHCN data analytics also play a role in identifying new health plan members, supporting health center member services to enroll and re-enroll members so they do not lose coverage.

Data analytics answer questions that directly relate to health access, quality and outcomes, such as:

  • Are you seeing patients enough?
  • What services are your patients using?
  • How are you using specialty care?
  • Which of your patients have been recently hospitalized?

The range of reports not only can trend the financial cost of care, they can alert providers to individual members who may have potentially serious health conditions. With regular data in hand, health centers can reach out to patients to help lessen the probability that they will develop a health condition or a negative health result because of their chronic conditions. Patients with high needs may even receive complex care services, including Care Neighborhood and Care Transitions.

Cost Benefit:

Acting on health data this way translates into better quality care for the member, but also generates cost savings that are reinvested to continue to improve health access, quality and outcomes. For example, data about patient hospitalization helps health centers work with the patient after they’ve completed an inpatient stay, avoiding a future stay or a readmission. Health centers also apply CHCN data analytics to prevent avoidable emergency room visits. Working closely with its IT division, CHCN continues to strive for ways to apply data analytics in service of our patients and providers.
