To log into the CHCN Provider Connection click here. If you do not have a user name and password, you will be asked to download and fill in a Request Access form.
Please note: Effective 3/17/14, CHCN has switched to an upgraded Provider Portal which has a new URL (internet address), design, but has all the same functionality with user friendly enhancements. Once you log on to the new Portal with your current credentials, you will be asked to update your password. New passwords must be at least 8 characters in length, must have at least one non alpha numeric character and cannot be a recent password.
