Community Health Center Network (CHCN) was formed by our member health centers in 1996 in order to more effectively participate in newly launched state managed care programs. The formation of CHCN was built upon 20 years of collaboration in health policy and advocacy work through the Alameda Health Consortium.

CHCN member community health centers are headquartered in Alameda County, California, with services provided to surrounding counties. Our health center corporations are: Asian Health Services, Axis Community Health, La Clinica de La Raza, LifeLong Medical Care, Native American Health Center, Tiburcio Vasquez Community Health Center, Tri-City Health Center, and West Oakland Health Center.
